Your accreditation application is important - checklist for completion

If you are soon due to submit an application for accreditation, please read the following information carefully. This will ensure that there are no delays or hiccups in processing renewal of your accreditation.

The Agency cannot proceed with an application unless it is valid. This is stipulated in legislation. To ensure that your application is valid, check the following points:

1.   The Agency approved application form is used and completed in all respects including administrative information;

2.      The appropriate fee is paid. For information on fees, click here;

3.      The home's organisational chart and its reporting links to the approved provider is included;

4.      Any information that you provide to residents at time of entry is included, i.e. information about your home and the services provided (for example, resident handbook, leaflets, etc);

5.      Site plan is included (for example, as given to residents or staff to orientate themselves to the building/s or fire evacuation diagram showing the position of the building/s);

6.      The home’s vision, values, philosophy, objectives and commitment to quality are included.
